Church Forms

In order to help streamline your interaction with the church, we will provide church forms online. We will usually provide three different ways to access the form:

• An Adobe Acrobat PDF file. This is an industry standard format that you probably already have access to on your machine. If you do not, go to Adobe Acrobat website and download and instal the FREE viewer. The instructions are included on their page. When a PDF file is displayed on your screen, the church website's menu WILL NOT appear. You may save the file to your local machine or print it out. When you are done, press the BACK button on your browser to return to the church web site. You may fax, mail or hand deliver the form to the appropriate church office.
• A Zipped Microsoft Word document. A "Zipped" document means that the document has been compressed to download faster (and also to correctly download to your machine). Your machine will usually have a copy of WinZip to uncompress the file. If not, please visit Winzip. You may download an evaluation copy. If you find the product useful you may wish to buy a licensed version. You may then print out the document and fax, email or hand deliver back. You may also complete the form online and email the file back to the church office.
• On-line submission form. This will direct you to a window in which you can fill in the information on line and SUBMIT the form to the church office. If the form requires additional information (such as a receipt) you may send this in separately.

Use this form to submit expenses over $50.00. The expenditure should be approved in the budget or by the appropriate person or committee.
